In order to recommend the most appropriate seating and wheeled mobility equipment for a client, it is essential that a thorough evaluation takes place. The Education in Motion Seating & Mobility Evaluation form is intended to serve as an example and guide as to what to include when evaluating a client for seating and wheeled mobility equipment.  As with the entire wheelchair provision process, this assessment is a team effort. The clinician, the client and others in the client’s support team should be involved.
